Ást Healthcare Services provides telehealth psychiatric evaluation and medication management for older adults throughout Delaware.
We welcome referrals for patients who may benefit from a careful psychiatric assessment, diagnostic clarification, medication review, or ongoing psychiatric medication management. Our practice has a particular focus on older adults with medical complexity, polypharmacy, cognitive or behavioral concerns, and psychiatric symptoms that require a thoughtful, individualized approach.
Appropriate Referrals
Referrals are appropriate for older adults with:
- Depression, especially in the setting of chronic medical illness or functional decline
- Anxiety disorders
- Bipolar spectrum disorders
- Sleep concerns associated with psychiatric conditions
- Behavioral and psychological symptoms associated with dementia
- Complex psychiatric medication regimens
- Polypharmacy or concerns about psychiatric medication safety
- Medication side effects, sedation, confusion, or medication-related fall-risk concerns
- Need for psychiatric consultation, diagnostic clarification, or medication-management follow-up
Services Provided to Referred Patients
- Comprehensive psychiatric evaluations
- Psychiatric medication management
- Review of medication effectiveness, tolerability, and side effects
- Medication-safety and polypharmacy review
- Consideration of medication simplification or deprescribing when clinically appropriate
- Collaboration with primary care clinicians, specialists, therapists, caregivers, and families, with patient authorization
- Referral to psychotherapy, community resources, or other specialized services when indicated
Referral Information to Include
When available, please include:
- Reason for referral and current psychiatric concerns
- Relevant psychiatric, medical, and cognitive history
- Current medication list, including prior psychiatric medication trials
- Recent relevant laboratory results, hospital records, or specialist notes
- Relevant caregiver, family, primary care, therapist, or specialist contact information
- Any current safety concerns or recent emergency psychiatric care
